God, yes -- there's nothing more irritating to have to keep flipping through a book to check references or the like. Especially frustrating in fiction.

Oh I love it! I'm not sure what I like more, the use of "endnotery" the threat to defame publishers on your blog or the request for cream paper and proper indexes. :)
